Applicants are required to submit three (3) color ID pictures taken within six months prior to the date of filing the application. Each photo measures 1.77" x 1.37"in size with plain white background and a clear view of the applicant's face. Applicants should be photographed in their decent attire. Eyeglasses should be removed. Wearing of earrings by male applicants is not allowed. Holders of old brown passports will need to submit documentary proof of his or her full middle name when renewing the said passport. 1. RENEWAL OF PASSPORT ???? ?????????????????????????????????????????? The Philippine passport is valid for five (5) years from the date of issuance. A holder of a Philippine passport is encouraged to apply for renewal of the passport at least six months before the expiration of its validity period or when its visa pages are about to run out. The requirements for renewal of passport are as follows: a) Application form duly accomplished; ????? b) Valid passport and photocopy of its personal data page and visa page; Seamen applicants are required to also submit a photocopy of their Seaman's Book and a letter of request or certification of employment from the ship's master ; c)?Three (3) color ID pictures (example in figure 1); d) Personal appearance; and, e) Payment of processing fee. (Subject to change). 2. FIRST APPLICATION BY FILIPINO CHILDREN BORN IN THAILAND A child born in Thailand to a Filipino father or mother can be considered a Filipino citizen?and may be entitled to a Philippine passport. The requirements for the issuance of a passport to the child are follows: a. Personal appearance of at least one parent; b. Application form duly accomplished; c. Original copy of the Birth Certificate of the child translated into English and duly authenticated by the Thai Ministry of Foreign Affairs; d. Three (3) color ID picture of the child. (examples in Figure 1); e. Original passports of the parents and four (4) photocopies of each passport; f. Original copy and four (4) photocopies of the marriage contract of the child's parents; (f.1) If the marriage was solemnized in the Philippines, the marriage contract must be duly authenticated by the National Statistic office (Office of the Civil Registrar General), Office of the President, Malaca?ang (Legal Office),and by the Department of Foreign Affairs (Authentication Division) (f.2) If the marriage was solemnized under Thai Law, the marriage contract must be accompanied by an English translation duly authenticated by the Thai Ministry of Foreign Affairs. g. Report of Birth form duly accomplished (Please refer to item III.1, for requirements); and h. Payment of processing fee (Please refer to the Schedule of Consular Fees). 3. REPLACEMENT PASSPORT A passport lost, damaged or destroyed in Thailand can be replaced. The applicant will be required to execute an affidavit explaining the circumstances under which such loss, damage or destruction of the passport occurred. A15-day waiting period commencing on the date of acceptance of the application is enforced to allow time for verification and possible recovery of lost passport, and for the Department of Foreign Affairs to notify its foreign service posts, concerned government agencies, and the diplomatic or consular corps about the cancellation of the passport reported lost and damaged or destroyed. The requirements for the application for replacement passports are as follows: a. Affidavit of Loss or Destruction duly notarized; In case of lost passport, original copy and photocopy of the Thai Police Report with English translation are also required. b. Application form duly accomplished. c. Three (3) color ID pictures (examples in Figure 1); d. Payment of processing fee (Please refer to the schedule of Consular Fees); and, e. Fifteen-day waiting period for lost passport.{mospagebreak}4. REVALIDATION OR EXTENSION OF VALIDITY OF OFFICIAL PASSPORT The Philippine Embassy may revalidate or extend the validity of an Official Passport, which is issued to Philippine government officials or employees who are in Thailand on official business. The requirements are as follows: a. Letter of Request from the applicant b. Valid passport; c. Photocopy of the passport's personal data and visa pages; d. Confirmation of request and approval from the Department of Foreign Affairs; and e. A certification from the sponsor stating that the application needs an extension of stay. 5. AMENDMENTS TO PASSPORT Amendments are authorized changes made to a Filipino's passport details as a result of a change in civil or legal status. An example is when a Filipino woman, after contracting marriage, decides to use the surname of her husband. Passports can be amended when a Filipino woman decides to revert to her maiden name after the death of her husband, after her marriage was annulled, or after being divorced by her foreign spouse. The passport can also be amended to reflect a change of surname of a child who is legitimated by virtue of a subsequent marriage of his parents or, in case of adoption, to reflect the surname of the child's adoptive father. The specific requirements are as follows: (5a) Amendment Due to Marriage 1. Application form duly accomplished; 2. Valid passports; 3. If marriage was solemnized in the Philippines, original copy and photocopyof the marriage contract duly authenticated or certified true copy by the National Statistics Office, Office of the President, and the Department of Foreign Affairs. 4. If marriage was contracted in Thailand. + Original copy and photocopy of marriage contract issued by appropriate Thai government agency translated into English and duly authenticated by the Thai Ministry of Foreign Affairs; + Original Copy of the Legal Capacity to Marry issued by the Philippine Embassy; and, + Report of Marriage form duly accomplished. 5. Payment of processing fee. (Subject to change). (5b) Amendment Due to Death of Husband 1. Application form duly accomplished; 2. Copy of the Death Certificate of the husband duly authenticated by the NSO, Office of the President, Department of Foreign Affairs. 3. If the Death Certificate was issued by a Thai government agency, the Death Certificate must have an English translation and must be duly authenticated by the Thai Ministry of Foreign Affairs; 4. Copy of the Birth Certificate of the husband duly authenticated by the NSO, Office of the President, and Department of Foreign Affairs; 5. Valid Passport; 6. Payment of processing fee. (Please refer to the Schedule of Consular Fees). (5c) Amendment Due to Annulment of Marriage 1. Marriage form duly accomplished; 2. Certified true copy of the Court Order effecting the annulment; 3. Annotated Marriage Contract duly authenticated by the NSO, Office of the President, and the Department of Foreign Affairs; 4. Valid passport; and, 5. Payment of processing fee. (Please refer to the schedule of Consular fees). (5d) Amendment Due to Divorce 1. Application form duly accomplished; 2. If the divorce was obtained under Thai law, original copy and photocopy of the divorce decree issued by a Thai District Office translated into English and duly authenticated by the Thai Ministry of Foreign Affairs; 3. If the divorce was obtained in other countries, a certified True Copy of the Divorce Decree duly authenticated by the Philippine Embassy or Consular Post which has jurisdiction over the place where the divorce was obtained (Filipinos are encouraged to file for amendment at the Philippine Embassy or Consular post which has jurisdiction over the place where the divorce was obtained); 4. In case of a woman divorced from her Filipino Muslim husband, certified true copy of the Divorce Decree or a certified true copy of the Certificate of Divorce from the Shariah Court or the NSO; 5. Valid Passport; 6. Payment of processing fee. (Please refer to the schedule of Consular Fees). (5e) Amendment Due To Adoption 1. Application form duly accomplished; 2. Valid passport; 3. Copy of the marriage contract of the parents duly authenticated by the NSO, Office of the President, and the Department of Foreign Affairs; 4. If the marriage was contracted under Thai law, copy of the marriage contract translated into English and duly authenticated by the Thai Ministry of Foreign Affairs 5. Payment of processing fee. (Subject to change). 6. USE OF SURNAME BY ILLEGITIMATE CHILDREN Foreign Service Circular No. 32-98 provides that illegitimate children shall use the surname of their mother only. Middle name is unimportant and its omission has no legal effect. However, illegitimate children may use the surname of the father if there has been an acknowledgment or recognition on the part of the father. The requirements for the use of the surname of the father are as follows: 1. Affidavit of Recognition by the father, duly notarized; 2. Passport application form, duly accomplished; and; 3. Payment of processing fee. (Subject to change). |
