V. Notarial Services

The most common notarial services include administering oaths and affirmations, jurat ( the memorandum giving the date and place of the taking of oath and the signature of the affiant), and the preparation of an affidavit

The general requirements for the notarization of documents are as follows:

a) Original copy and two (2) Photocopies of the document to be notarized;

b) Original passport of the applicant and photocopy of the passport's personal data and visa pages;

c) Personal appearance; and,

d) Payment of processing fee (Subject to change).

2. AUTHENTICATION

When an officer of the Thai government has performed a notarial or other act for use in the Philippines, the official character of the executing officer should be verified by the officer of the Philippine Embassy. The officer of the Philippine Embassy shall then issue a Certificate of Authentication in order to give the service legal force and effect in the Philippines.

The requirements for the issuance of a Certificate of Authentication are as follows:

a) Original copy and photocopy of the document duly authenticated by the Thai Ministry of Foreign Affairs; and,

b) Payment of processing fee ( refer to the Schedule of Consular Fees).

3. ACKNOWLEDGMENT

An acknowledgment is the act by which a person who has executed a legal document goes before a competent officer and declares it to be his true and voluntary act and deed. When a person who has executed a legal document appears personally before an officer of the Philippine Embassy and is acknowledge thereof, the officer shall execute a Certificate of Acknowledgment. The requirements for the Issuance of a Certificate of Acknowledgment are as follows:

a) Original and copy of the legal document executed;

b) Personal appearance; and,

c) Payment of processing fee (Subject to change).
